Somewhere in year five her sister told her to start taking B12. No particular reason — her sister took it, it was on sale, it seemed like a sensible thing for a tired person to do. She bought it and she took it and she never thought about it again.

Because metformin depletes B12. Steadily, over years, in a way nobody feels happen. And low B12 looks like fatigue, brain fog, and tingling in the hands and feet — which get filed under age, under menopause, under the diabetes itself. Almost anything except the pill that caused it.

Metformin interferes with how B12 is absorbed in the last stretch of the small intestine. The effect builds with dose and with years on it, and a meaningful share of long-term users end up low.
What makes it slippery is that low B12 and long-standing diabetes cause overlapping symptoms — fatigue, fog, and tingling or numbness in the hands and feet. So the symptom gets attributed to the condition rather than the treatment, and the actual cause is a blood test away.
The mechanism is certain. The consequence is argued.
Statins work by blocking an enzyme early in the cholesterol pathway — and CoQ10 is built further down that same pathway. Blocking the top lowers both. That part isn't in dispute; blood CoQ10 measurably drops.
What's genuinely argued is whether replacing it does anything for the muscle aches statins sometimes cause. Trials go both directions. Anyone selling you certainty here is selling.
The one people take for years without meaning to.
Stomach acid is how you free B12 from food. Proton pump inhibitors — omeprazole, pantoprazole and the rest — reduce that acid on purpose, and over long stretches B12 absorption falls with it.
Magnesium is the sharper issue. Long-term PPI use can drive magnesium low enough that the FDA requires a warning about it, and low magnesium shows up as cramps, palpitations and poor sleep long before anyone thinks to check.
These were designed for short courses. Plenty of people have been on one for a decade because nobody ever said stop.
Started at twenty, still taking it at forty.
Combined oral contraceptives are associated with lower folate, B6 and B12, and often lower magnesium and zinc as well. The drops are usually modest — but they run for years, and they overlap with mood and energy complaints that get attributed to almost anything else.
Worth knowing rather than worth panicking about. And genuinely worth knowing for anyone thinking about pregnancy, where folate status matters most.
None of this is a reason to stop taking anything. Not one of these drugs is on this page because it's bad — they're here because they work, which is why people stay on them long enough for a slow drain to matter. Stopping a prescription over something you read online is a far bigger risk than any depletion on this page. The move is a conversation and a blood test, not an empty bottle.

You don't need to argue with anyone or arrive with a theory. You need one sentence: "I've been on this a long time — can we check my levels?" That's a reasonable request and most prescribers will say yes without blinking.
For metformin or long-term acid reducers, that's B12 — and worth knowing, serum B12 can read normal while you're already functionally low, so methylmalonic acid is the more sensitive follow-up if symptoms don't match the number. For acid reducers, magnesium too. For anyone on hormonal birth control, folate and B6.
